By registering a child for programs offered by Havilah Soccer Academy / Havilah Creative Programs ("Havilah"), the parent or legal guardian acknowledges and agrees to the following terms:
Participation in soccer training, physical activity sessions, and creative workshops involves inherent risks, including but not limited to falls, collisions, cuts, strains, and other minor or serious injuries. Parents and guardians voluntarily accept these risks on behalf of their child.
To the fullest extent permitted by law, parents and guardians release, waive, and discharge Havilah, its owners, instructors, volunteers, and staff from claims for injury, loss, or damage arising from program participation, except where caused by gross negligence or willful misconduct.
Parents and guardians confirm the child is fit to participate and will disclose relevant medical conditions, allergies, medications, or injuries during registration. In an emergency, Havilah staff may provide basic first aid and seek medical treatment when required, and emergency contacts will be notified as soon as reasonably possible.
Parents and guardians are responsible for timely drop-off and pick-up. Only individuals authorized by the parent or guardian may collect the child. Havilah will not release a child to unauthorized persons.
All participants are expected to behave respectfully and safely. Havilah may suspend or remove any child whose conduct creates risk or serious disruption. Where appropriate, this may be done without refund in line with program policy.
Havilah may request separate media consent for photos or videos used in website, social media, or marketing materials. Parents and guardians may grant or decline media consent without affecting registration eligibility.
Program fees are due as communicated at registration. Refunds, credits, and transfer requests are handled according to Havilah's current cancellation and refund policy.
By selecting the required waiver checkbox on the registration form and submitting registration, the parent or legal guardian confirms they have read, understood, and agree to these terms.
